1. For Grades 8, 9, 10, 11 and 12: Two (2) photocopies of School ID.
2. For Incoming Grade 7: Two (2) copies of Parent ID and School ID during elementary.
3. For All Applicants: Duly accomplished form with an attached 2x2 picture.
NOTE:
- 1 student/beneficiary per family. Bawal ang magkapatid.
- Student ang magpapasa at kukuha ng form.

STEP 1: Prepare and complete all the necessary requirements (Requirements 1 or 2, depending on your grade level).
STEP 2: Bring your requirements and a 2x2 picture to the Multi-Purpose Hall to claim the application form.
STEP 3: Fill out the form completely and submit it to the assigned SK Council member.
NOTE: Kapag mekumpleto na pu ing slots, misara ya pung diretsu ing pamagpasa. Pakatandanan, "๐๐๐๐๐ ๐พ๐๐๐, ๐๐๐๐๐ ๐๐๐๐๐" pu ini.
