Partnered Pharmacist Medication Charting: Innovating Medication Safety
Operations Directorate

Medication errors are a significant patient safety risk, with traditional medication charting models linked to high error rates. In response, the Pharmacy Departments at St George Hospital and Prince of Wales Hospital, in partnership with their Emergency Departments, introduced Partnered Pharmacist Medication Charting (PPMC).

PPMC pairs a credentialled pharmacist with a senior medical officer at admission to develop and co-sign a patient’s medication plan, enabled by recent NSW legislative changes.

This innovative model has reduced medication errors from up to 60% to around 3.3%, while also improving efficiency, patient safety, and overall care outcomes.

This Bowel Cancer Awareness Month we urge eligible people living in South Eastern Sydney to take the bowel screening test, with only 39 per cent of those who receive the kit in the South Eastern Sydney Local Health District taking the test.

The test is available to those aged between 45 and 74 years and is the easiest way to detect the early signs of bowel cancer, Australia’s second deadliest cancer. If caught early, bowel cancer can be successfully treated in more than 90 per cent of cases.

People aged 50 to 74 receive free bowel screening tests every two years, sent to the address they have registered with Medicare. People aged 45 to 49 years need to request their first test kit. They will automatically receive kits every two years after completing their first one. You can request a kit here: https://www.ncsr.gov.au/request-a-free-bowel-test-kit

The MyTest HIV self-test machine is now available at St George Hospital!

🗺️Where?
You can find our MyTest machine outside the Emergency Department at St George Hospital, on Hospital Way near the corner of Gray Street.

⏲️When?
All day every day! Yep, you can access the MyTest machine 24/7.

🤔How?
To access a test, register online at https://mytest.health.nsw.gov.au/ and then visit the machine to collect your test at a time that suits you.

Instructions are available in multiple languages, along with information on what to do if your result is positive for HIV, as well as how to access prevention and support services.

🤷‍♂️Why?
Because we want to improve access to free and confidential HIV self-testing, that’s why!

A major milestone was reached this week with the first patient treated in the new biplane room at the St George Hospital, Kensington Street Building.

Mr Tan underwent a cardiac diagnostic angiogram to investigate a suspected blocked artery identified on a prior CT scan. The results were encouraging, with no blockage requiring a stent.

“I feel very lucky to be the first patient in the new building. I wish I was this lucky with Lotto,” Mr Tan said.

This procedure marks the clinical commencement of the Philips biplane system, a state-of-the-art imaging platform shared by Interventional Radiology and Cardiology. A biplane system uses two X-ray cameras at different angles at the same time, giving clinicians highly detailed, real-time views of the heart and blood vessels.

Designed for flexibility, the system supports a wide range of interventions, from minimally invasive procedures to complex cases, helping improve workflow, enhance outcomes and deliver better care for patients.

A significant step forward in expanding advanced procedural capabilities at the Kensington Street Building.

OUR PEOPLE | “I come originally from Condobolin in Central New South Wales. At university, I did a science degree and I loved anatomy and physiology. I came to Orthoptics after finishing my science degree.

At the moment I am the TeleVision project manager, as Sydney/Sydney Eye Hospital (SSEH) are developing a new virtual model of care to support emergency ophthalmology cases in rural and regional emergency departments (ED’s). We provide eye cameras for their ED’s and then those photos are accessible here at SSEH for our TeleVision project ophthalmologists, who can then have a video consult. Some local health districts don't have ophthalmology services, even on call, so they're left in the dark. If they can't manage patients locally, then we know exactly what we're dealing with and we're getting them straight here, straight to surgery, whatever we need to do.

The Sydney Eye Hospital Foundation (SEHF) has generously provided a grant for the pilot project. We have Wollongong Hospital as our first pilot site, but we've had a lot of interest across the State. We're doing a phased approach to see how our pilot goes and then hopefully expand from there.

Most people don’t know that I used to do salsa and bachata dancing, and I even performed. I don't do it at the moment. I dropped off during COVID and then I didn't really get back to it, but I was obsessed.
