Patients with incurable cancer often face complications that significantly impact quality of life. Image-guided interventional radiology offers minimally invasive palliative treatments to relieve symptoms, reduce tumour burden, and manage complications such as obstructions and fluid collections. This topic package explores these supportive therapies and highlights the importance of tailoring treatment approaches to each patient’s individual needs.

𝐓𝐡𝐚𝐧𝐤 𝐘𝐨𝐮, 𝐂𝐈𝐑𝐒𝐄 💖

As this week’s takeover comes to an end, I would like to sincerely thank CIRSE & ISVIR India for this opportunity to share a glimpse of Interventional Radiology practice from New Delhi, India.
It has been a privilege to showcase not only the diverse spectrum of cases we treat, but also how we approach patient care, teamwork, decision-making, and training in our daily practice.
I am grateful to my mentors, trainees, nurses, technologists, and support staff who contribute to the care of our patients every day.
❤️‍🩹 Most importantly, thank you to the patients who place their trust in us and allow us to be a part of their journey.
Thank you to everyone who followed along throughout the week.

👋 Greetings from New Delhi, India.

Acute limb ischaemia (ALI) is a time‑critical vascular emergency where clinical decisions must be made quickly, often under imperfect information and with limited margin for error. Modern endovascular tools allow rapid restoration of patency in many patients, but outcomes depend on more than reopening a large artery - distal embolization, spasm, no‑reflow, and reperfusion injury can still drive tissue loss.

This Topic Package provides an overview of the contemporary endovascular management of acute limb ischaemia, covering patient assessment, imaging, thrombolysis, thrombectomy, and adjunctive treatment strategies. It also explores complication management, multidisciplinary decision-making, and the importance of selecting the most appropriate treatment approach for each individual patient.

Acute ischemic stroke is one of the few medical emergencies where every minute has a direct impact on patient outcomes.

While mechanical thrombectomy has transformed the treatment of large-vessel occlusive stroke, successful stroke care begins long before the first catheter enters the vessel.

At our institution, a dedicated Code FAST is activated as soon as a patient with suspected acute stroke arrives in the Emergency Department. Rapid clinical assessment, expedited imaging, prompt decision-making, and seamless coordination between emergency physicians, neurologists, radiologists, nurses, technologists, and the neurointerventional team allow patients to move efficiently from diagnosis to treatment.

The images below represent a selection of stroke thrombectomy cases managed through this pathway—from vascular occlusion to successful reperfusion.

For endovascular specialists, stroke care is a reminder that outcomes depend not only on technical expertise, but also on efficient systems of care, multidisciplinary teamwork, and timely intervention.
