Transitioning Together

Transitioning Together Our goal is to support them in building meaningful connections and a strong support system that creates future opportunities for themselves and their families.

TT fosters community engagement by hosting monthly activities bringing together adults with special needs and the broader community to build connections, share new experiences, and create a supportive, inclusive network for all involved. We are dedicated to fostering inclusion for the special needs community by helping individuals develop social skills that empower them to not only integrate into society but to thrive.

Is it bad that almost all of our posts start out with being happy, grateful, overjoyed, thankful, excited, or whatever other positive emotion you want to add?

I'm glad it's not, because I am still smiling about our most recent eventโ€”Karaoke & Line Dancingโ€”which may have been one of the best of 2026. ๐Ÿ’™

Art Rob The Heart Throb was amazing, the dance floor was packed, and the music was poppin'! TT participants filled the Larema Coffee House! ๐Ÿค ๐Ÿ•บ๐Ÿ’ƒ๐ŸŽค๐ŸŽถ

Friends from middle school were reunited and new friendships were formed. The smiles were EVERYWHERE! ๐Ÿฅฐ๐Ÿฅฐ๐Ÿฅฐ

So many reasons to be thankful.

Stephenson Millwork, thank you for your sponsorship and for helping make this event possible.

Thank you to our volunteers for leading the dances and keeping the energy high all night long.

Kevin at Larema Coffee House, thank you for allowing TT's version of a coffee house "takeover" be so successful. We so appreciate your support of inclusion in our community.

Most of all, thank you to all the families and caregivers for believing in the TT mission and coming out to dance and sing with us.

Nights like these remind us why we do what we do. ๐Ÿ’™

๐Ÿ’™ Caregiver Tips ๐Ÿ’™

Building Independence One Step at a Time

As caregivers, it's natural to want to help. Sometimes, though, the greatest gift we can give is the opportunity for someone to do things for themselves.

Building independence doesn't happen overnight. It grows through practice, patience, and encouragement. Here are some ideas to try out.

โœจ Allow extra time for tasks.
โœจ Offer choices whenever possible.
โœจ Encourage problem-solving.
โœจ Celebrate effort, not just success.
โœจ Remember that mistakes are part of learning.

What may take a little longer today can lead to greater confidence tomorrow.

Every small step toward independence helps build self-esteem, decision-making skills, and a stronger sense of accomplishment.

At Transitioning Together, we believe in creating opportunities for adults with special needs to grow, connect, and thrive while being supported every step of the way.

๐ŸŒŸ Progress isn't perfection. Progress is growth.

๐Ÿ’ฌ What's one independent skill your loved one has developed that makes you proud? We'd love to celebrate with you in the comments!

โšพ๏ธ What a memorable day at the Wilson Warbirds Ball Clinic! โšพ๏ธ

Our hearts are full of gratitude after such a wonderful experience this past weekend.

A couple of our Transitioning Together participants had never played baseball before, and watching them step onto the field, learn new skills, build confidence, and have so much fun was truly special. The smiles, laughter, encouragement, and excitement throughout the day reminded us why inclusive opportunities matter so much.

A huge THANK YOU to Olivia with the Wilson Warbirds and Kelsey with Wilson Parks & Recreation. Both were absolutely incredible! Their kindness, patience, generosity, and commitment to inclusion made every participant feel welcomed, valued, and successful.

Kelsey, we have a feeling you may have a few new Miracle League players joining you this fall! ๐Ÿ˜Š

We are especially honored that Transitioning Together was personally invited to participate in this event. Opportunities like these create meaningful experiences, new friendships, and lasting memories for our participants and families.

Thank you to everyone who made this day possible. From the bottom of our hearts, thank you for seeing the abilities in every individual and creating a place where everyone belongs.

๐Ÿ’™ Together, we are building a more inclusive communityโ€”one experience at a time.

๐ŸŒŸ Friday Encouragement ๐ŸŒŸ

Progress isn't always measured by big milestones.

Sometimes progress looks like:

๐Ÿ’™ Trying something new
๐Ÿ’™ Walking into an unfamiliar room
๐Ÿ’™ Saying hello to someone new
๐Ÿ’™ Participating in an activity
๐Ÿ’™ Showing up when it would be easier to stay home

Every small step builds confidence.

Every connection matters.

Every effort deserves to be celebrated.

Whether you're an individual with special needs, a family member, caregiver, volunteer, or supporter, remember that growth happens one step at a time.

Keep moving forward. We are cheering you on!

๐Ÿ’™ Have a wonderful weekend from all of us at Transitioning Together.

๐Ÿ’ฌ What is one thingโ€”big or smallโ€”you are proud of accomplishing this week? We'd love to celebrate with you in the comments!

๐Ÿ“Š Did You Know?

Many adults with intellectual and developmental disabilities experience increased social isolation after graduating from high school. Once school-based programs and activities end, opportunities for social connection often become much more limited.

At Transitioning Together, we believe everyone deserves meaningful friendships, opportunities to participate in their community, and a place where they feel welcomed and valued.

That's why we create inclusive events where adults with special needs, families, caregivers, volunteers, and community members can come together, build relationships, and experience the joy of belonging.

A bowling game may seem simple. A karaoke night may seem small. But friendships are formed, confidence grows, and lives are enriched through these shared experiences.

Together, we can build a community where no one feels left out and everyone knows they matter.
