CIVILIAN VS MILITARY TRAUMA CARE: THE PHASE NAMES ARE DIFFERENT

Tactical Emergency Casualty Care uses its own phase language because it describes a different operating environment — civilian patients, civilian scope, civilian scene control. The phases are Direct Threat Care, Indirect Threat Care, and Evacuation Care, governed by the Committee for Tactical Emergency Casualty Care, not the military's CoTCCC. If someone's teaching civilian response using military phase names, ask which framework they're actually working from.

Hands-Only CPR exists because the mouth is what stops most people from acting, not the hands.

The American Heart Association removed the mouth from the equation for one situation: a teen or adult who suddenly collapses.

✔️ Call 911 first.
✔️ Heel of one hand center chest, stack the other on top, lock elbows, push with body weight.
✔️ Target: at least 2 inches deep, 100 to 120 compressions per minute — the tempo of songs you already know.
✔️ Keep going until they wake up, an AED arrives, or professionals take over.

Put gloves on.

Turn the lights down.

Have someone yell.

Practice tourniquet application and hemorrhage control from the position
you'd actually be in — seated, prone, behind cover.

The gap between how you train and how it happens in a real emergency is the gap you'll fall into.

THERE IS NO NATIONAL AGE THAT DEFINES "PEDIATRIC" IN FIELD TRIAGE⁠

People assume there's a fixed pediatric age cutoff — twelve, fourteen, sixteen — written into some national trauma standard. There isn't. ⁠

When the National Expert Panel on Field Triage revised its field triage guideline in 2021, it deliberately chose not to define pediatric patients by a fixed age, because the evidence doesn't support one and because EMS systems already set their own age limits based on local resources and practice patterns. That means the real answer to "is this patient pediatric" lives in your protocol and your regional trauma plan — not in a number half-remembered from a first aid class. ⁠

Check your system's pediatric age cutoff before you need it. Most responders never have.⁠
