The Chemawa Indian Health Center provides a range of patient-centered family practice outpatient medical and dental services. Our clinic is conveniently located in Salem, Oregon on a beautiful campus surrounded by a variety of trees, birds, and a wetland. Our services are available to all federally recognized American Indian and Alaska Native people and their descendants living in or visiting the service area. Services Include:

• Diabetes Education and Support (1 on 1 or group setting)
• Pre-Diabetes and Diabetes Self-Management Class

Public Health Nursing

• Full service in-house tests available to providers within 1 hour
