🤔Did you know, every 5 minutes, an older adult in New Jersey is seen in the emergency room because of a fall?

In 2024, 85% of fall-related hospital admissions involved adults age 60+, with nearly half age 80 or older. The average hospitalization cost for a non-fatal fall exceeded $123,000, and the total cost of hospital-treated falls among older adults surpassed $4 billion statewide.
These statistics serve as a powerful reminder that fall prevention and home safety are critical to helping older adults remain safe, healthy, and independent, at home.

Have you ever felt like you were trying to solve a puzzle without having all the pieces?

For many families, that’s exactly what navigating a healthcare diagnosis, hospital discharge, aging concerns, or a loved one’s changing needs can feel like. There are so many decisions to make, resources to understand, and questions that often go unanswered, and it doesn’t stop there.

I hear this combined question the most often is: “I don’t know what this is, please explain it and how can you help me?” “What are you going to do to make it easier for me?”

Care Navigation helps families understand their options. It provides guidance, education, and direction through the often-complicated healthcare and aging services systems.

Care Management builds on that foundation by helping coordinate care, communicate with providers, advocate for needs, and ensure that plans are put into action.

When these two approaches are combined, families receive both the knowledge they need to make informed decisions and the support and assistance they need to carry them out.

My goal is not simply to provide answers—it’s to help families understand their choices, feel confident in their decisions, and know they have someone walking alongside them through the process.

Because when families are informed, supported, and empowered, they are better together. 💜🌸
