The Office of Inspector General was formed to identify and investigate persons who commit TennCare fraud or abuse, to coordinate efforts with other agencies, as well as to recover money owed to the state. The OIG does not take this mission for granted and wholeheartedly works alongside other agencies to preserve those resources for
their intended use. As with any worthy program, there are those who take advantage of the same opportunities through questionable means. (Tennessee Code Annotated 71-5-2505). It is imperative they be accurate, timely, and thorough. The OIG, in collaboration with local, state, and federal agencies, as well as healthcare providers, identify, investigate, and seek prosecution of individuals who defraud the TennCare program. Support for the OIG mission on the part of local law enforcement, prosecutors, healthcare providers, citizens, and members of the General Assembly has been a tremendous asset. As a result, the OIG’s resolve to stop TennCare fraud is even stronger. To report TennCare recipient fraud or abuse, you may use any of the following methods:
•Call the Fraud Toll Free Hotline at 1-800-433-3982. You may also call the Office of Inspector General’s main telephone at 615-687-7200 to be transferred to a Hotline staff member.

•Download and complete the “Report Recipient Fraud” form below and e-mail the attachment to [email protected]
•Print and fax the applicable downloadable form below to 615-256-3852.
