Did you stop by the Long Island Cares food truck at Mercy Haven? 🚚

We were on-site connecting with Medicaid members and families and sharing information about how HEALI connects members with services that can make everyday life healthier, safer, and more stable, from good food and asthma support to home repairs, heating and cooling assistance, and more.

These services are part of your healthcare plan and, if you qualify, may be available at no cost to you.

It’s Community Health Worker Awareness Week! 🎉

But what exactly is a CHW? CHWs are people from your community who work at local organizations or doctor’s offices. They listen to what’s making it harder to take care of your health and provide practical, hands-on help connecting you to Medicaid services you may qualify for, all at no cost to you because they’re covered by your health plan.

Maybe that’s healthy food delivered to your home to help manage diabetes. Maybe it’s air conditioning to help your child breathe easier when they have asthma.

CHWs meet you where you are, especially when things get hard.
