We are thrilled to share a major milestone for A Place To Go CLE. We are formalizing a collaborative partnership and referral workflow with Anthem Blue Cross and Blue Shield (Ohio Medicaid – SDOH Whole Health) to provide comprehensive supportive services for members across Cuyahoga and Lake County.
True wellness extends far beyond the clinic walls. By bridging the gap between managed care healthcare systems and grassroots community action, our shared goal is to help members navigate Social Determinants of Health (SDOH), remove barriers to care, achieve their employment goals, and build long-term financial stability.
Through this streamlined ecosystem, we are bringing our core service strengths directly to the table to shift acute, high-cost crises into stable, preventative, and routine care:

Where: Cuyahoga County Probate Court. Cuyahoga County Court House, 1 W Lakeside Ave, Cleveland, OH 44113

Trained volunteer attorneys will be available to help you complete forms and discuss your questions concerning legal name change, birth certificate correction, and more from
10:00am until 3:30pm!

Notaries will be available! Probate magistrates will also be able to process filings and issue orders on site!

Licensed professionals will be on hand to help with professional statement forms!

There are costs associated with filing fees:

$100 for name change and
$47 for birth certificate correction
Financial assistance will be available for those who need it. Applications will be taken on site.
What to bring:
Government-issued photo ID
Copy of birth certificate

Cash or money order for the filing fee(s)
For gender marker corrections, a licensed professional statement is needed. Bring this form completed in your current legal name, or day of licensed professionals will be on hand to help:

Cuyahoga County resident for at least 60 days

For gender marker correction:

Proof that you were born in Cuyahoga, in some instances a long form birth certificate may be needed.

For Ohio birth certificate correction:

Cuyahoga County resident, OR born in Cuyahoga Meanwhile, in Geauga County

In need of a donated 24 inch, single speed bicycle tire for our Harmony Road Bicycle program. An unsheltered individual will be provided a bicycle and pull along trailer once completed, which will extend their range and facilitate easier access to employment, appointments and more.
