House panels use “emergency” to boost NIH, DOE science budgets

This week, spending panels in the U.S. House of Representatives began voting on bills to fund the government, and a few of them made use of an emergency mechanism to beef up research budgets. The National Institutes of Health (NIH) and the national laboratories funded by the Department of Energy’s (DOE’s) Office of Science were the big winners, receiving a combined additional $11.25 billion. But to date, the other federal research agencies have come away empty-handed.
The panels were operating under a tight spending cap adopted by Congress last year for the 2021 fiscal year that begins on 1 October. It limits the rise in overall spending on domestic discretionary programs, which includes all nonmilitary research, to $5 billion over 2020 levels. That’s less than 1% in a budget of $627 billion.
But lawmakers retained the option of exempting some activities from the cap by declaring them to be emergency spending. With the arrival of COVID-19, that safety valve also provided a mechanism for science advocates to argue for larger research budgets to help universities deal with disruptions caused by the global pandemic. Last month, a bipartisan group of legislators in the House of Representatives introduced a bill, the Research Investment to Spark the Economy (RISE) Act, that would authorize spending $26 billion across several federal research agencies to do exactly that.

The National Institute of Allergy and Infectious Diseases (NIAID), part of the National Institutes of Health, has established a new clinical trials network that aims to enroll thousands of volunteers in large-scale clinical trials testing a variety of investigational vaccines and monoclonal antibodies intended to protect people from COVID-19.
The COVID-19 Prevention Trials Network (COVPN) was established by merging four existing NIAID-funded clinical trials networks: the HIV Vaccine Trials Network (HVTN), based in Seattle; the HIV Prevention Trials Network (HPTN), based in Durham, N.C.; the Infectious Diseases Clinical Research Consortium (IDCRC), based in Atlanta; and the AIDS Clinical Trials Group, based in Los Angeles. Those individual networks will continue to perform clinical trials for HIV vaccine and prevention and other infectious diseases in addition to their new COVID roles.
“Establishing a unified clinical trial network is a key element of President Trump’s Operation Warp Speed, which aims to deliver substantial quantities of a safe, effective vaccine by January 2021,” said HHS Secretary Alex Azar. “Starting this summer, this new network will leverage existing infrastructure and engage communities to secure the thousands of volunteers needed for late-stage clinical trials of promising vaccines.”
“Having a safe and effective medical countermeasure to prevent COVID-19 would enable us to not only save lives but also help end the global pandemic,” said NIAID Director Anthony S. Fauci, M.D. “Centralizing our clinical research efforts into a single trials network will expand the resources and expertise needed to efficiently identify safe and effective vaccines and other prevention strategies against COVID-19.”
The network’s vaccine testing will be led by Larry Corey, M.D., of the Fred Hutchinson Cancer Research Center in Seattle, and Kathleen M. Neuzil, M.D., M.P.H., of the University of Maryland School of Medicine. The network’s monoclonal antibody clinical testing efforts will be led by Myron S. Cohen, M.D., of the University of North Carolina, Chapel Hill, and David S. Stephens, M.D., of Emory University in Atlanta. The HVTN, which is based at the Fred Hutchinson Cancer Research Center, will serve as the COVPN’s operational center

As NINR director, Dr. Zenk will oversee NINR’s annual budget of nearly $170 million, the large majority of which supports extramural research at institutions across the Nation. NINR science seeks to improve the lives of individuals and families living with illness and to develop personalized strategies to maximize health and well-being at all stages of life, and across diverse populations and settings. NINR’s intramural program on the NIH campus conducts research to better understand and manage symptoms. Within both of those programs, NINR devotes significant resources to training and career development to foster the next generation of nurse scientists.

Dr. Zenk’s own research focuses on social inequities and health with a goal of identifying effective, multi-level approaches to improve health and eliminate racial/ethnic and socioeconomic health disparities. Her research portfolio also includes NIH-supported work into urban food environments, community health solutions and veterans’ health. Through pioneering research on the built environment and food deserts, Dr. Zenk and her colleagues helped bring national attention to the problem of inadequate access to healthful foods in low-income and Black neighborhoods.

The NIH supports $31 billion in research annually, given to more than 300,000 researchers at more than 2,500 institutions for research into a variety of conditions.[2] Each institute of the NIH has separate appropriations from Congress determined on an annual basis. Percentages of grant applications funded vary by institute, from 8% (National Institute of Nursing Research) to 29.6% (National Institute of General Medical Sciences), with an overall average of 18%. Funding percentages have dropped from over 30% in the early 2000s, mainly due to an increase in applications, rather than a decrease in funds available. In 1998, 24,100 applications were received, and 7,500 were funded with a total of $1.9 billion. By 2005, the number of applications had grown to 43,000, of which 9,600 were funded with a total of $3.4 billion.
