❓ What if the student did not complete the intake form at the DSWD?
✔️ You will still receive your assistance during the payout, but you are required to go to the DSWD Office on Monday to complete your intake form.

❓ What if the student cannot attend due to emergency, OJT, or work?
✔️ The student may send a representative. Please bring:
• Authorization Letter (signed by the student)
• Valid ID of the authorized representative
• Valid ID of the student
