CALL FOR SCHOLARSHIP APPLICANTS
Educational Scholarship Program of LifeBank Foundation & LGU-Santa Barbara
The LifeBank Foundation (LBF), in partnership with the Local Government Unit of Santa Barbara, is pleased to announce the 1st year of implementation of the Educational Scholarship Program for incoming 1st Year College Students.
Who are Qualified:
1. Must be a BONA FIDE resident of the Municipality of Santa barbara, Iloilo.
2. Must come from an indigent family but NOT a beneficiary of the Pantawid Pamilyang Pilipino Program ( 4Ps ).
3. Must be a recognized member of the Indigenous Peoples (IPs) in the Municipality of Santa Barbara, Iloilo. (If applicable)
4. The applicantโs parents must NOT be members of the Ikabuhi Microfinance Program (IMP).
5. Must have a GENERAL AVERAGE of at least 85% (With Honors) in Grade 12.
6. Must be enrolled in any recognized College or University within the Province of ILOILO.
7. Must NOT be a beneficiary of any government scholarship. (DOST, CHED, TES or the like)
Documentary Requirements:
I. Certified True Copy of Grade 12 Report Card.
II. Authenticated Birth Certificate (PSA)
III. Authenticated Marriage Contract of Parents (PSA)
IV. Certificate of Good Moral Character (from Barangay)
V. Certificate of Indigency (from Barangay)
VI. Certificate of Non-4Ps Membership (from MSWDO)
VII. Medical Certificate (from Rural Health Unit, confirming that the applicant is free from any chronic illness)
VIII. Long Brown Envelope (with Name, Address and Contact Number on the upper right corner)
Application Period (Submission of Requirements):
August 15, 2025 to September 10, 2025
