While SSU-PC OSAS prepares the necessary documents, students listed below are encouraged to prepare their requirements now to help ensure a smooth and hassle-free claiming process.

🎓 FOR ENROLLED STUDENTS:
*Six (6) colored photocopies of your validated school ID, front and back on the same page;
*Each copy must bear three (3) fresh signatures

🎓 FOR GRADUATES:
*Six (6) colored photocopies of a valid government-issued ID, front and back on the same page;
*Each copy must bear three (3) fresh signatures

👨‍👩‍👧 IF CLAIMING THROUGH A PARENT AS AUTHORIZED REPRESENTATIVE:
*Six (6) original copies of a notarized Authorization Letter;
*Six (6) colored photocopies of the grantee’s ID, front and back on the same page, each with three (3) fresh signatures;
*Six (6) colored photocopies of the parent’s valid ID, front and back on the same page, each with three (3) fresh signatures

👤 IF CLAIMING THROUGH A NON-PARENT (ex: sibling, spouse) AUTHORIZED REPRESENTATIVE:
*Special Power of Attorney (SPA)
*Six (6) colored photocopies of the grantee’s ID, front and back on the same page, each with three (3) fresh signatures
*Six (6) colored photocopies of the authorized representative’s valid ID, front and back on the same page, each with three (3) fresh signatures.

For enrolled students:
