1๏ธโƒฃ Prepare three (3) sets of the correct COC form, printed back-to-back on Folio (8.5 x 13) or Legal-size copy paper
2๏ธโƒฃ All copies must bear the candidate's ORIGINAL signature
3๏ธโƒฃ Attach a Passport-size photo taken within the last six (6) months on each COC
4๏ธโƒฃ Attach one (1) P30.00 Documentary Stamp Tax or two (2) P15.00 DST
5๏ธโƒฃ Attach 3 Copies of PSA or LCR-issued- Original, Certified True Copy, Printed Copy, or Photocopy- all accepted.
6๏ธโƒฃ COC must be Notarized/under oath and bear the seal and signature of the Notary Public
7๏ธโƒฃ Candidates have the option to attach a Bio-data and Program of Government
8๏ธโƒฃ All fields must be completed. Write "N/A or Not Applicable" where appropriate.
The filing period is set from September 28, 2026 to October 5, 2026 ONLY!
