Upang hindi maabala at mabilis na ma-encode ang inyong aplikasyon, pakidala ang mga sumusunod:
PARA SA MGA OPERATORS:
๐๏ธ Driver's License ( Professional ) Operator at Driver
๐๏ธ Barangay Residency Certificate at RBI Certification
๐๏ธ Valid Franchise / Notice of Award (Dapat ay aktibo at hindi expired)
KUNG REPRESENTATIVE ANG MAG-PROSESO:
๐ Special Power of Attorney (SPA) o Authorization Letter mula sa mismong Operator.
๐๏ธ Photocopy ng Valid ID ng Operator at ng Representative.
๐ก PAALALA SA MGA APPLICANT:
Siguraduhing malinaw ang mga photocopy ng inyong dokumento.
Dalhin din ang Original Copies para sa verification/validation purposes.
Magsuot ng maayos na kasuotan at sumunod sa mga health at safety protocols sa loob ng Municipal Compound.
