|| REQUIREMENTS: BUSECO Application of Membership & Electric Service

✅2x2 Picture with white background
✅BUSECO Application of Membership form
✅Photocopy of latest Community Tax Certificate (Cedula)
✅Barangay Clearance or Residence Certificate
✅Original Electrical Permit/ Mayor’s permit issued by LGU with official receipt
✅Certificate of Attendance for the BUSECO Pre-Membership Education Seminar
✅Electrical Sketch Plan or Blueprint; signed and sealed by P*E and LGU-OBO
✅Photocopy of either a valid Government ID with birthdate or Voter’s Certificate (with 3 signatures)
✅Proof of Ownership (lot title; lease contract; or notarized authorization of lot owner
✅Original Certificate of Completion for house wiring installation issued by BAEA
for residential and REE-RME-P*E for non-residential building
✅TIN
✅Waver of non-objection from neighboring lots for service drop wire traversing other properties with ID from signatory (BUSECO Form)
✅Authorization to tap from the owner of a private transformer/ steel pole or Barangay
✅Photocopy, photo, or details of your nearest neighbor’s power bill for reference
