Distance does not reduce concern. This week, we had a call from a daughter who lives abroad. She had visited mum and recognised deterioration.

We allocated an experienced Care Coordinator who is available 7 days a week, 12 hours a day to visit, monitor, advocate and address any care issues or practical problems that go wrong in the home, reducing the daughter's anxiety. We are her eyes and ears now.

Rewarding work, being able to offer peace of mind to family who do not live near.

It’s more common than you might think. Someone goes into hospital and from there into a care home, often as a temporary measure. However, for those who are self-funding and without immediate support around them, returning home can feel overwhelming or even impossible.



At My Care Coordinator, we assess each individual’s situation, looking at what’s possible. If a safe return home can be achieved, we will put the right care and support in place to make it happen.



To date, we’ve supported 8 individuals to successfully transition from a care home back to their own home, each with the guidance of one dedicated Care Coordinator.
